The Challenge
After being discharged from the hospital, Lani, 49, was living alone and skipping meals. While physical recovery was challenging, coping without support made it even harder. Her strength was declining, and so was her morale.

We evaluated Lani for food insecurity and social isolation, scoring her at Moderate Risk (score: 14). She was matched with a mobile pantry and received assistance with SNAP enrollment. Within 36 hours, pantry partners confirmed delivery, and more importantly, Lani felt supported and acknowledged.
Results
Lani’s recovery was jeopardized, but access to food and connection helped turn things around. KokuaNet ensured that care continued beyond hospital discharge.
