KokuaNet is a closed-loop referral platform that helps identify and resolve social determinants of health (SDOH) barriers by matching people to local, verified services — and confirming when care is received.
A gap is considered closed when a local organization accepts the referral and the patient confirms they received the service. You only pay when the loop is verified.
Pricing is based on the acuity level of the individual, determined by their screening score.

No. You’re only charged if the referral is accepted and the patient confirms receipt of service. This ensures full accountability.
We calculate risk using a research-backed scoring system. Each social factor (like food insecurity or unsafe housing) is weighted by its impact on health outcomes. Scores are tiered into Low, Moderate, and High risk.
Yes. We provide metrics like time-to-service, gap closure rates, re-trigger rates, and estimated avoided costs per tier — helping you show both outcomes and savings.
No subscription or onboarding fees. Our model is purely pay-for-performance.
If a patient says they didn’t receive services — or doesn’t respond — we retry alternate matches in the area until a gap is successfully closed.
We confirm care in two ways: first, the community-based organization (CBO) must respond “yes” to the referral and provide a service date. Then, we follow up directly with the patient—via SMS, phone call, or caregiver check-in—to verify that services were actually received. If the patient says “no” or doesn’t respond, we re-trigger the referral process until a confirmed match is made.
A loop is considered closed when the referral is accepted by a provider and the patient confirms that they received services. If either fails, we continue matching until it’s closed.
If there’s no response within 36 hours (or 8 hours for urgent cases), KokuaNet automatically re-triggers the next best-fit provider until someone says “yes.”
